Psychological Safety Assessment:
Identifying What's Really Holding Your Team Back
Our Psychological Safety Assessment provides a comprehensive, confidential review of your work group's dynamics to identify barriers to performance and deliver a clear roadmap for building a more engaged, collaborative, and effective culture.

If you're an HR business partner, you probably have one: a team you've been managing incident-by-incident for what feels like forever, putting out today's fire without ever getting near what's causing it. It's exhausting, and it quietly erodes credibility even when you're doing everything right. The leaders who bring us in often admit that it's not even the issues they already know about that worry them most, it's what they don't yet know about.
Our assessment is how we find that out. It's a structured look at what's happening in the team, the recurring perceptions underneath what you've been managing incident by incident, rather than another round of guessing at symptoms. Our assessment does not assign blame for what's already happened. Instead, we give you a clear, prioritized way to move the team somewhere better.
This is proactive work, not a response to a specific incident. We capture how employees actually perceive their workplace, and mirror those perceptions back anonymously to everyone who took part, alongside our recommendations for shifting the problematic perceptions toward something more positive. From there, we work directly with the leadership team to prioritize those recommendations and chart initial actions, so the report becomes something acted on, not something that sits on a shelf.
Our structured methodology includes:
Data Collection. We gather qualitative data through document reviews, written surveys, confidential individual interviews, and focus groups.
Analysis & Reporting. We analyze the data to identify common problematic perceptions, determine their causes, and prepare a detailed report with clear recommendations.
Debriefs. We present our findings, anonymized, to everyone who took part, along with our recommendations for shifting problematic perceptions toward more positive ones. We then work with the leadership team to prioritize those recommendations and chart initial actions, so the report leads to real change rather than sitting on a shelf.

Frequently Asked Questions
Is this only for teams already in open conflict? Not necessarily, and often it's the opposite: teams stuck in a long-running, exhausting pattern rather than an open conflict are exactly who this is built for. If a specific conflict or investigation has already happened, Restorative Conferencing or Post-Investigation Restoration may be the more immediate fit. Will individuals be identified in the findings?
No. the interviews are private, and our reports do not name individuals or provide other identifiers. The goal is to mirror back how the group talks about their workplace and what to do about it, not to attribute specific comments to specific people.
How long does the assessment take, from data collection to debrief?
Typically about four weeks in total, though it depends on scheduling availability and the number of employees involved.
What happens after the debrief, do you help implement the recommendations?
Yes. After the debrief, ProActive meets with the decision-makers to support their prioritization of our recommendations and land on initial actions, so they don't just sit on the shelf.
